Hagerstown and surrounding area residents are serviced by our center. Please understand that payment of your bill is part of your treatment and care.

Plastic Surgery Services participates with many insurance companies. Please call the office to see if we participate with your insurance when you make your appointment. Our office will file insurance for all reimbursable services, to both your primary and secondary insurance carriers. Please remember that you are responsible for all deductible, co-pay, and non-covered service amount. All cosmetic procedures are not covered by your insurance.

Is the patient responsible for obtaining any medical records, previous test results, MRI report, etc. prior to the appointment?

Yes, Patient is responsible for obtaining their own medical records prior to their office visit. The reason for obtaining these records prior to your appointment is so that Dr. Garazo can completely review your medical records prior to your consultation.

CareCredit works just like a credit card, but is exclusive for healthcare services. And, it’s better because you get no interest financing or low monthly payments every time you use it. You can use your card over and over for follow-up appointments or different procedures. This means you don’t have to put your health and beauty on hold until you save up enough money.

If Dr. Garazo recommends surgery, you will see his surgical coordinator. They will answer specific questions about the surgery scheduling process, surgical fees, discuss the paperwork and tests involved, and complete all pre-certifications/authorizations if your insurance company requires it.

Plastic surgeon, Hagerstown, Dr Garazo requires that a parent or legal guardian MUST accompany patients who are minors. This accompanying adult is responsible for payment of the account, according to the policy outlined below.

HMO with which we are not contracted .	Payment in full for office visits and other charges at the time of office visit.	File an insurance claim on your behalf.
Point of Service Plan or Out Of Network PPO	Payment of the patient responsibility (deductible, co-pay, non-covered services) at the time of the visit.	File an insurance claim on your behalf.
Medicare	Any services not covered by Medicare are requested at the time of the visit. If you have Regular Medicare as primary and also have secondary insurance or Medigap : No payment is necessary at the time of the visit.	File the claim on your behalf, as well as any claims to your secondary insurance.
No Insurance	Payment in full at the time of the visit.	Work with you to settle your account. Please ask to speak with our staff if you need assistance.
